Understand How to Identify Eligible Employees
There are three ways to identify eligible employees for the National DPP lifestyle change program.
Using the Prediabetes Risk Test might be best for you if you:
- Want to raise awareness broadly among your employees.
- Prefer that employees be able to opt-in to the program without someone reaching out to them first.
- Have upcoming health screenings or fairs.
- Can reach a good portion of your workforce through some form of health communication.
- Plan to administer the benefit through your health plan, pharmacy benefit manager, or other third-party administrator.
- Have strong data systems to easily run queries either internally or through a vendor.
- Intend to use claims-based reimbursement and need to connect identifiers.
- Work with your on-site health clinic to track health improvement.
- Already have strong partnerships with the clinical community.
- Are hosting upcoming health screenings or fairs.
- Have a higher-than-normal rate of turnover or have recently hired a significant number of new employees.
