Our Story
The Centers for Disease Control and Prevention (CDC) and the National Association of Chronic Disease Directors (NACDD) have been working with State Health Departments and national organizations to explore how we can help expand coverage and support for type 2 diabetes prevention among employers.
CDC and NACDD have provided collaborative learning experiences, intensive trainings, and other technical assistance to teach employers how to offer the National Diabetes Prevention Program (National DPP) lifestyle change program as a covered benefit. During this process, they identified a need for a decision support tool that could reach more employers more efficiently.
We applied what we learned from working with over 300 employers to create this dynamic tool. Healm will help advance coverage of the National DPP lifestyle change program in your workforce. As you embark on your Healm journey, trained experts are ready to support to you.

National Association of Chronic Disease Directors (NACDD)
NACDD is a nonprofit, public health organization that includes chronic disease program directors from all 50 U.S. states, 5 U.S. territories, and 3 freely associated states. It also includes 7,000 chronic disease professionals working in state, tribal, and territorial health departments, nonprofits, academia, and the private sector. Since 1988, NACDD Members have worked to strengthen state-based leadership and expertise for chronic disease prevention and control across the United States.

The Division of Diabetes Translation is part of CDC’s National Center for Chronic Disease Prevention and Health Promotion. The division established and oversees the National DPP, which is a partnership of public and private organizations working to build a infrastructure to support delivery of an evidence-based, cost-effective lifestyle change program to prevent type 2 diabetes in adults at high risk.
